Living a Healthy Life with Chronic Conditions
Do you or a loved one suffer from a chronic condition such as heart disease, arthritis, diabetes, or emphysema? If so, do you find your chronic condition hard to manage at times? Then attending this Chronic Disease Self-Management Workshop is key to managing your chronic condition more effectively. This is a six-week program designed to help people with one or more chronic diseases to manage their condition and begin living a more active and fulfilling life. You will learn how to: manage symptoms, set weekly goals that are achievable, work with your doctor, and include fitness and exercise. This workshop is free to participants and all will receive a free workbook and “Time for Healing” relaxation CD.
