Thank you for your interest in joining the St. Elizabeth Patient and Family Advisory Council (PFAC) where we strive to be the voice of the patient.

If you have a passion for improving the experience of St. Elizabeth patients and their caregivers; and if your experience has been within the last two years, please consider filling out an application to join our council.

What is a PFAC?

Patient and Family Advisory Councils (PFACs) bring together patients, families, staff and clinicians to enhance the experience for all people, evaluate strategies and improve quality and safety outcomes. All members of the Patient & Family Advisory Council are either current or former patients and/or caregivers who are passionate about improving the lives of patients at St. Elizabeth Healthcare. We come together to participate in the development and evaluation of St. Elizabeth programs, services, and policies.

PFAC Mission

The Patient & Family Advisory Council exists to support the mission of St. Elizabeth Healthcare, which states, “As a Catholic healthcare ministry, we provide comprehensive and compassionate care that improves the health of the people we serve.” Specifically, the Council exists to ensure the very best for patients and families as acting as advisors for the St. Elizabeth Healthcare team to support the creation of an environment of safety, quality, and service.

The vision of the Patient & Family Advisory Council to be an innovative leader in raising the standard of excellence for patient and family centered care.

What is expected of PFAC Members?

  1. CORE COUNCIL Member

    • Involves the commitment of attendance at one two-hour meeting every other month. Meetings will be virtual apart from two in-person meetings at the half and full year.
      • At these meetings, a St. Elizabeth associate will present an idea and seek our feedback and opinions based on your personal experiences.
      • You may be asked to lead or participate in one of the council’s special projects. Occasionally, you may be asked to volunteer to represent the PFAC at various meetings within St. Elizabeth.

Application Process

  1. Applicants are asked to complete the following application. For questions or concerns, please email PFAC@stelizabeth.com.
  2. Applicants will be contacted by a Co-Chair of the PFAC within two business weeks after receipt of their application.
  3. Following the review of your application, you may be asked to take part in a short phone introduction with up to 3 current PFAC members.
  4. Applicants who are invited to join the Core Council must then register with St. Elizabeth’s Volunteer Office and complete any current St. Elizabeth Healthcare Volunteer requirements (such as vaccinations, flu shots, brief online training, etc.)

Our 2024 PFAC is limited to 20 members who reflect and represent the demographics of the patients of St. Elizabeth Healthcare. This enables us to fully inform the organization about the experiences and opinions of its patient population.
