Patients will continue to receive cancer treatments from their oncologist while simultaneously receiving support and care at our Advanced Palliative Cancer Care Clinic. This collaborative approach puts patients at the center of care with specific focus on reducing suffering and increasing quality of life. Visiting the Clinic When patients first come to the clinic, they will meet with our Nurse Navigator who will review their medical records and talk about their comfort level and goals. We use evidence-based screening tools and questionnaires in our assessment to help determine the right care for each patient. Based on the patient’s individual needs, we will then refer them to the specific caregivers who can provide medical care, psychosocial, spiritual and emotional support.
