At St. Elizabeth Healthcare, we utilize the American Medical Society for Sports Medicine’s (AMSSM) Universal Application for Fellowship Training.

To supplement your application, we request:

• One of your letters of reference be from your residency program director, covering your performance in residency and overall abilities in Family Medicine.
• At least one letter of reference from a physician knowledgeable about your potential for excellence in musculoskeletal medicine.
• You include your In Service Training Exam results, and, if applicable, your Family Medicine Board Certification Exam completion date and results.

Please Note: All applications must be received by October 1, 2012.
